Become an Oak Street Health patient by scheduling your first visit with one of our primary care providers. At your new patient appointment, you will get to know your provider and care team, share your medical history, undergo an initial clinical exam, and sit down with your Medicare resource specialist.

    Register as a new patient

    Submit a new patient registration form to book your appointment.

    Confirm your details

    Once booked, we’ll call you to confirm your Medicare insurance, address, and date of birth.
